Heart attacks, bypass surgeries, angioplasty, angina and other cardiac-related conditions can be terrifying for you and your family. But there is no reason to stop living life to the fullest.


At Pattie A. Clay, we help you overcome your fears and help prevent problems and additional surgeries through a progressive, three phase cardiac rehabilitation program.

By learning how to live a heart-healthy lifestyle, including modifying your diet and integrating exercise into your daily routine, your heart won't have to work so hard any more. Caring for your heart is a lifelong process and Pattie A. Clay has a program designed to meet your needs every step of the way.

The three-phase program includes:
Phase 1 - Inpatient Education: Our goal during Phase 1 is to help you stay well and avoid future hospital visits. While you're staying with us, your cardiac care team will introduce you to several steps you can take upstream -- before another cardiac incident occurs -- to help your heart pump more effectively and to improve the quality of your life.

Phase 2 –Beginning Outpatient Cardiac Rehab Program - This is a safe, medically monitored outpatient program designed to help participants reduce stress, improve quality of life, build healthier hearts and resume most, if not all prior activities. Exercise, education, stress management and support are the main program components, and a physician referral is needed. The program begins within two to six weeks following the patient’s discharge from the hospital. It involved three sessions per week on Monday, Wednesday and Friday. The sessions usually lasts approximately one to one and one-half hours and are offered five times per day, three morning and two afternnon sessions.

The program lasts from six to twelve weeks, depending on the number of visits authorized by the patient’s insurance provider.

The program uses a variety of exercise equipment to condition and maintain heart endurance with close staff supervision and telemetry (EKG) monitoring. Patients also have access to private lockers, towels, juice-water and snacks. Patients also take part in educational classes with the various treatment team members on a variety of topics specifically designed for them.

Phase 3 - Medical Fitness-Maintenance: This supervised exercise program is for people who have a chronic health condition or prefer medical supervision and is designed to bridge the gap between the outpatient cardiac rehab program and the start of a home fitness regimen. Program participants benefit from the support of registered nurses and exercise physiologists who can personalize exercise regimens. All regimens include blood pressure and pulse monitoring, as well as warm-ups and cool-downs. Once a patient feels comfortable on their own, they may choose to exercise at home or continue with Phase 3 in our Cardiac Rehab Center. Insurance companies do not usually provide benefits for Phase 3 of Cardiac Rehab and patients are charged a nominal monthly fee to continue their exercise regimen at our facility.
 
The Cardiac Rehab Center provides:

  • EKG monitored exercise sessions
  • Medication review
  • Dietary counseling
  • Risk factor modification
  • Stress management/relaxation techniques
  • Educational sessions with multidisciplinary staff members
  • A support network to interact with other cardiac patients

How do Patients Enroll in Cardiac rehab? A referral from a physician is required. Any physician, not necessarily a cardiologist, can refer a patient. If you are interested in cardiac rehab, ask your physician or have them call us for more information about our program. The Cardiac Rehabilitation Center is open Monday through Friday from 7:30 am to 4:30 pm.  

The Cardiac Rehabilitation Center is located at the Pattie A. Clay Regional Medical Center on the second floor of Medical Office Building #1, Suite #26. Mob #1 is directly across from the Emergency Room. There is plenty of free parking available around the building, with an elevator just inside the door for your convenience. The building is handicapped accessible. Please call for assistance, if needed.
