The health information management department (HIM) (medical records) is a support service of Pattie A. Clay Regional Medical Center. Its primary purpose is to contribute to the quality of patient care through the development and maintenance of a comprehensive, centralized medical records system. The HIM department ensures that the hospital has accurate and complete medical records for each patient admitted and uses required guidelines to ensure that medical records are accurate, timely, complete, accessible and secure.

Copies of medical records may be picked up between 8 a.m. and 4:30 p.m. Monday – Friday.
You may call 625-3152 to make an appointment to pick up medical records. Please allow 72 hours advance notice and bring a picture ID. The first copy of your medical record is free but after the first copy the charge will be $1.00 per page.
